Description
Poste : Nous recrutons un(e) Infirmier(e) Case Manager pour renforcer notre nouvelle équipe " Réhab Jeunes Adultes ".
Vous coconstruirez, avec le patient, son projet personnalisé à partir d'échelles spécifiques et vous serez le/la référent(e) de son parcours de soins.
Vous coordonnerez les interventions et les activités qui permettront d'atteindre les objectifs identifiés.
Vous utiliserez les outils de réhabilitation psychosociale : projets personnalisés, psychoéducation, entraînement aux habiletés sociales, stimulation cognitive, travail en réseau, inclusion des familles (programme BREF), etc.
Vous intègrerez une équipe engagée composée de 3 soignants par jour (2 le week-end) pour un service de 28 lits d'hospitalisation complète.
Vous travaillerez en équipe pluridisciplinaire : médecins psychiatres, médecins généralistes, psychologues cliniciens, neuropsychologue, assistante sociale, ergothérapeute, APA, coach job, pair-aidant, etc.
Le poste, en contrat à durée indéterminée, à temps complet, est à pourvoir immédiatement.
Profil : Titulaire du Diplôme d'Etat d'Infirmier, vous disposez d'une première expérience en psychiatrie.
Vous appréciez le travail en équipe pluridisciplinaire.
Vous disposez de compétences relationnelles, organisationnelles, et avez le sens de l'anticipation.
Vous êtes autonome, curieux(se), bienveillant(e) et engagé(e).

Entreprise : La Clinique de Beaupuy est un établissement de 160 lits de psychiatrie générale. Située à 12 km de Toulouse et implantée dans un parc de 7 hectares, l'environnement permet d'offrir aux patients un cadre agréable et reposant pour bénéficier de soins de qualité.
Dotée de 5 unités de soins libres, 1 spécialisée en soins sans consentement, elle bénéficie également d'un service d'hospitalisation à domicile. L'établissement, dynamique dans ses projets, a récemment obtenu une autorisation d'hôpital de jour qui ouvrira ses portes au centre de Toulouse dans un nouveau bâtiment en cours de réalisation. La clinique a à coeur de promouvoir un projet médical tourné vers le patient acteur de son soin, sur le principe de la réhabilitation psycho-sociale, portée par des professionnels investis et engagés.
